Please fill out the following form to apply for Research Wizard membership.
Membership requires payment of an annual fee of $120.
As a member of Research Wizard, you will receive a discounted hourly rate of $50 over Research Wizard's regular $60 per hour charge.  Research Wizard's minimum per-project charge of $60 will also be waived when the service is used more than once a month.
Additionally, you will receive Research Wizard Business Brief, which will alert you to new business information sources.

Upon receipt of this form, Research Wizard will contact you for payment arrangements.  You will also receive a Membership Plan packet.  We appreciate your interest and we look forward to serving your information needs.
